HOW TO OBTAIN A LIBRARY CARD

**Due to popular demand, at this time new library cards will be issued to new library card applicants, renewal applicants and those who have lost their card (a $1 surchange for lost cards will apply)
Library cards are free of charge to residents and non-resident taxpayers of the Lindenhurst School District. Current proof of residency or ownership must be shown to be issued a library card. Your library card provides you with direct access to many materials in all of Suffolk County's public libraries. Please present one of the following forms of identification to receive your library card. Note: All ID must be current and in the individual's own name and address. A post office box is NOT acceptable ID.
Adults
- Driver's License
- Car registration or insurance card
- Utility bill
- Legitimate correspondence (examples: bank statement, credit card statement, medical statement, etc.)
- Cancelled check with address imprint (A checkbook listing the address is not acceptable)
- ID card from the Department of Motor Vehicles
- Tax bill or property ownership bill
Students
- Parent/Guardian's ID
- Driver's Permit
- School Schedule
- School ID/Bus Pass
- Report Card
Person's under 18 years of age must have a parents or guardians signature on the library card application. The replacement cost for a lost or damaged card is $1.00 per occurrence. Current identification is required. When renewing a card, current identification must be shown with your existing Lindenhurst Memorial Library card.
